LifeLinks provides an individualised whole-of-life approach to service provision. We work closely with you and your family/whānau to develop an Outcome Plan and support strategies to achieve your identified goals.

We take a whole-of-life approach and work beside you to review key life areas to identify what’s working well, what can be improved and what natural, mainstream and funded supports might assist you to achieve your goals.

LifeLinks recognises that family/whānau and carers play a central role in the lives of people with disabilities and that accessing support at an early stage is essential for maintaining wellbeing.

Ask your outcome coordinator about services, supports and agencies that provide families/whānau/carers with support, information, advice and social networks.
